Our Approach

The process of commencing care with Complete Complex Care involves six stages. When arranging care, we follow a clear process to ensure that you receive personalised and high-quality service. Our consistent approach ensures excellent care no matter where you live.

This is what our service users can expect when we commence a care package:

  • Receiving your enquiry, or a referral from healthcare organisation on your behalf
  • A local specialist from our team will promptly reach out to you upon receiving your inquiry to discuss the provision of care and gather some initial information.
  • If service users are looking to pay for care privately, or the care will be funded through case management, then we will provide an outline of costs.

We'll use a collaborative approach to help individuals achieve their personal well-being goals. We do this by:

  • Empowering people to have choice and control over their care provision, ensuring they are
equal partners in the planning process.
  • Work with local communities to ensure sustainable services in their locality.
  • Providing a flexible service built around personal aspirations and wishes of our service users
  • Ensure that we operate a manageable and financially viable service provision with our
    partner commissioners.
  • Utilising a Preventative Approach with effective contingency planning and crisis intervention
    planning where necessary.
  • We work in partnership with multi-disciplinary teams.
  • We Focus on complex needs to ensure both short term and long– term planning is adequate
    to meet individual needs.
  • We look at what is important to people to ensure their personal aspirations and well-being is
at the centre of all we do.

We'll schedule visits by our Manager and nurse to meet our service users and their close relatives or household members in their own homes. We'll take the time to understand their needs, gather information for their personal profile and care plan, and conduct any necessary risk assessments during the visit.

With this information we will make a recommendation about the suitable nature and frequency of care to meet your needs. If you are a private client we will confirm the costs of the proposed care package. We will then include all this information in a formal care plan which we will discuss with you by phone or in person. We will answer any of your questions or those of your family and ensure that you are happy with every aspect. You have a final say in what is included in your care plan, unless this responsibility is awarded to a relative of guardian.

After your approval the staff who met you will identify a selection of Complete Complex Care client care staff with the skills and aptitude to support you in-line with your care plan. We always offer to arrange for you to meet these care staff in a ‘meet and greet’ prior to the start of your care. In some instances we may book additional staff for the first few visits, such as a nurse to supervise the start of your care package. And, we will run additional training, relevant to your needs, with our care staff.

Our Client Care Manager will make frequent calls and visits in the first two weeks of your care provision. Before your care starts, you will know who to expect and when they will be coming to see you. In most cases you will have met the care staff who will be providing care.

We'll review your care package six weeks after it begins and regularly afterward. If your needs change or there are staff changes or observations, we'll update your care plan after discussing it with you.

At Complete Complex Care, we prioritise achieving the best outcomes for our service users through a flexible and responsive approach. Our goal is to provide care that preserves each individual's independence, safety, and well-being. We promote robust contingency plans through well-managed information and outcome measurement systems. Emphasising a preventative care model, our operational approach focuses on meeting the needs of individuals and their families locally with efficient management support.

To suspend a care package due to a service user's hospitalisation, respite, or any other reason, contact the coordinator or on-call senior using the number provided in the service user guide. For non-private contracts with a care manager from the local authority, we'll inform the relevant authorities to adjust the care package. We require a minimum of twenty-four hours' notice for care withdrawal, but appreciate as much notice as possible. Charges still apply for less than 24 hours' notice. When care is reinstated and financed by the local authority, the care manager should contact Complete Complex Care to resume services.
